Pay Forms for Clubs

Online pay forms are available for all Simon clubs. Please start by contacting your club advisor and please be ready to answer the following questions:

Please allow one week for the product(s) to be added to the shopping cart.

  • Has Simon Finance Department been contacted?
  • What is the OP number?
  • What is the full name of the event?
  • Are there any specific instructions/details about the event that need to be on the page?
  • How many events will this pay form be for?
  • Will transportation be involved?
  • What is the cost (individually for multiple events or as one cost)?
  • What do you want the stock set to (unlimited or set number)?
  • Are there food options?
  • Do you want to collect information for an "Emergency Contact"?
  • What are the fields you want to collect? Examples:
    • First Name
    • Last Name
    • Email
    • Phone
    • Program of study
  • Who will need to be notified when a purchase is made (via Email)? Please list all Email addresses
  • Who will need access to the downloadable spreadsheet of purchases? Please list all Email addresses.
